Care Monitor - Case Manager Supervisor/Contract Monitor (LMSW)

The individual(s) who fills this position demonstrates that he or she is capable of carrying-out the following roles and responsibilities: 

  • Monitor compliance with the contract.
  • Assess and evaluate symptomatology referrals and treatment modalities for working with mentally ill adults, mentally ill children, developmentally disabled adults and children, and substance abusers.
  • Monitor treatment plans and treatments goals.
  • Actively participate in the Quality Improvement Process(es).
  • Coordinate services with other treatment team members.
  • Confer with and counsel providers regarding program and casework problems.
  • Counsel clients in groups or individually on experimental or planned basis.
  • Review and compare productivity or work performance of subordinate providers with established standards.
  • Participate in the development and implementation of administrative policy.
  • Train new providers in areas of departmental procedures, agency policy, and government or agency regulations.
  • Coordinate activities between service and unit providers.
  • Propose recommendations for changes in program and establishing unit policies, goals and procedures.
  • Assists providers through group and individual conference to examine case management issues in order to improve provider’s facilitation and diagnostic skills.
  • Provide in-service training for the experienced providers in areas of updated policy, procedure, and regulation.
  • Analyze productivity measures to ensure compliance with contract.

Required Skills: 

  • Ability to acknowledge diagnoses per symptomatology and/or DSM 5
  • Knowledge of treatment modalities
  • Knowledgeable in evaluating treatment activity, collaboration with enforcement agencies and court system.
  • Knowledgeable in treatment planning principles and self-determination principles.
  • Knowledge of Social Security and DHS benefits application and how to access them.
  • Have knowledge about local community housing and low income housing, and of local resources agencies and services they offer.
  • Ability to meet deadlines and work under pressure
  • Team player

Qualifications and Education Requirements:

  • Possess at least a master’s degree or higher in social work
  • Must be licensed by the state of Michigan: LMSW
  • Must have credentials of CMHP, QMHP, and QIDP
  • 18 years of age or older
  • Working telephone
  • Possess a valid driver’s license
  • Pass all regulatory checks
