The individual(s) who fills this position demonstrates that he or she is capable of carrying-out the following roles and responsibilities:

  • Projects a favorable image of the company to promote and enhance public recognition and acceptance of all of its areas of endeavor
  • Orient consumers into treatment programs and complete appropriate initial intake and first service paperwork, including consents/releases, relapse prevention plan, input for IPOS and treatment plan
  • Develop a Narrative Summary to diagnose per the DSM (*most updated version)
  • Knowledgeable in evaluating treatment activity, collaboration with law enforcement agencies and court system
  • Ability to assess and evaluate symptomatology, referrals and treatment modalities for working with co-occurring disorder, intellectually disabled and children with special needs
  • Monitor the tracking mechanism for timeliness of all intakes and first service appointments from date of referral to actual assessment completion
  • Agree to meet the consumer at their residence, community or at the office
  • Schedule initial appointments for psychiatric evaluations, therapy and case management within the required timeframe
  • Maintain good relationships with Community Mental Health (CMH) referral sources, other network providers, and CMH Quality Management Team
  • Coordinate with the CMH to obtain court orders as needed
  • Collaborate with other colleagues and/or Departments to assign consumers upon intake and or development of treatment plan based on medical necessity
  • Provide Community Resources when medical necessity does not exist
  • Refer and assist with SOAR program for social security income
  • Complete Medicaid applications
  • Provide initial case management contacts to begin person-centered planning, crisis/relapse prevention plan, and treatment plan
  • Provide warm transition to assigned case manager
  • Link, coordinate, and monitor referrals to appropriate community resources/services
  • Meet productivity standards as determined necessary to meet consumer needs; this includes the inclusion of initial and annual psychosocial assessments for case management consumers
  • Provide and crisis intervention services as determined medically necessary
  • Obtain and maintain cultural competency on the job
  • Completion of IPAR assessments as needed to refer to crisis services
  • Perform additional job duties as assigned

Qualifications and Education Requirements:

  • 18 years of age or older
  • Working telephone
  • Possess a valid driver’s license
  • Pass all regulatory checks
  • Must obtain required annual corporate trainings
  • Possess reliable Personal Transportation
  • LMSW or LPC with MHP and CMHP credentials
